All forms can be completed by typing in fields and checking boxes. We recommend using the current version of Acrobat Reader.

Please be advised that at this time digital signatures are not acceptable on forms. Therefore, you are required to print the form and sign it using ink. Save the form as a PDF document or a JPEG and upload the form to your SFP Student Portal.

2023-2024 FINANCIAL AID FORMS

FOR THE ACADEMIC YEAR 2023-2024 AND SUBSEQUENT ACADEMIC YEARS, FORMS MUST BE UPLOADED INTO YOUR FINANCIAL AID SFP PORTAL. WE NO LONGER RECEIVE PAPER VERIFICATION DOCUMENTS BY HAND, EMAIL, OR FAX. PLEASE MAKE SURE YOU CONTACT YOUR FINANCIAL AID REPRESENTATIVE OR CONTACT OUR OFFICE AT 850-599-3730 IF YOU HAVE ANY QUESTIONS OR CONCERNS.(SCANNED DOCUMENTS ONLY - PICTURES OF DOCUMENTS ARE NOT ACCEPTABLE) 

OUT-OF-STATE WAIVER APPLICATION

The Division of Student Affairs is pleased to announce the opening of the Out-of-State Student Fee Waiver (OSW) application for the fall 2023 and spring 2024 semesters. Please complete the application in full (do not omit any questions). Priority will be given to students with a 3.25 or higher GPA, an average of 15 credits completed per term, and who have filed their FAFSA by January 1, 2023. The OSW is a merit-based award with a need-based component. 

The application will open for 30 days (from March 1 – 31). The final determination of eligible students will be made by May 1, 2023.

 Student must be eligible to receive federal student aid and meet the following criteria:

  • Cumulative FAMU GPA of 3.25 or Higher
  • 2023/24 FAFSA filed by January 1, 2023
  • All relevant documents submitted to financial aid by April 1, 2023
  • Completed a minimum of one academic year at FAMU
  • Average 15 completed credits at FAMU per academic year completed
