Our Services

SHS provides outpatient primary care and health promotion services to students at FAMU. We are located on the first floor in the FAMU Center for Access and Student Success (CASS) Building, with an onsite pharmacy that provides easy access and discounted prices for prescription and over-the-counter medications. Clinic visits for immunizations, illness and injury are available on a walk-in basis. Appointments are required for other services. Student Health Services (SHS) has a primary care clinic that provides medical treatment and prevention services to registered students. Eligible students have access to unlimited clinic visits and reduced costs for other services.

Eligibility and Payment

Students currently enrolled for classes are charged a student health fee along with their tuition.  This health fee entitles students to access clinic and mental health services.

Health Insurance

Students are required to provide proof of insurance coverage or purchase the university-sponsored health insurance plan.  Students that fail to provide documentation of insurance coverage and students whose opt-out waivers are denied will be enrolled in the university-sponsored plan.

Immunizations

Florida Statutes establish immunization requirements for students attending state colleges and universities. Students are required to complete and submit information on the FAMU Health History and Immunization form.

Pharmacy

The Student Health Service Pharmacy is operated by the College of Pharmacy and Pharmaceutical Sciences.  Students may obtain prescriptions and over-the-counter medications as well as various health and beauty aids.

Medical Records & Confidentiality

    Medical Records

    Students may request a copy of their medical records for themselves or to be sent to another medical provider.  You can find an Authorization for Release of Information form below.  Contact the Office Manager at 850-599-3777 for more information.
    If you have a chronic illness or need follow up care for a recent injury or surgery, please request that your current health care provider send us a copy of your records or, at a minimum, a treatment summary that includes your medication history.
    This information may be faxed to the clinic at (850) 412-5643 or brought with you to your first clinic visit.
  • Confidentiality
    Confidentiality

    Treatment records for students age 18 and older are confidential information and are maintained according to state and federal laws.  Disclosure of information, including patient status, date and time or nature of visit requires written consent from the student, except in cases of emergency or as required by law.  Consent and authorization to release information must be specific about what information is to be released and to whom.  Without consent, information cannot be released to anyone, including parents, friends, instructors, or other persons.

After Hours Care

For urgent medical problems, students are encouraged to seek treatment at a community walk-in clinic or urgent care center.

If you are having a medical emergency, dial 911 or go directly to the nearest hospital emergency room.
