Service Request Guidelines
Service Requests are important tools in the operation the Office of Alumni Affairs. This office provides a number of services for alumni and other University departments. In order to ensure that these services are rendered in a timely fashion, we require the the requestor filll out a Service Request Form. The information that follows describes the kinds of requests that can be made as well as a few guidelines for making such requests. This form can be submitted electronically or it be printed and faxed.
University Departments and organizations may request data related to their particular areas or special events coordinated with our division
all requests for data or statistics must be made in writing on a Service Request Form
include detailed information about what you are requesting
if you are not an authorized party requesting information, your request may require approval before processing
