The director or other authorized officer must submit the Group Verification Request Form to the Registrar’s Office. This form can be downloaded from the Registrar’s webpage. To access the form go to http://www.famu.edu/index.cfm?Registrar&Forms. This is for FAMU inter-department use only.

Procedure:

          Step 1: Download the Group Verification Request Form.

          Step 2: Complete and sign the top part of the form.

          Step 3: Enter student’s full name and student Id.

          Step 4: Forward completed form to the Registrar’s office [Room 103 FHAC] or email to Registrar@famu.edu . Please note verification will be returned only to a FAMU email address
